1. All of the airlines you mentioned give relatively good service. Jet Blue is very good if they serve the city you want to go to. US Airways is late quite often. Southwest drastically overbooks every flight so you might get "bumped" off the plane even if you have a confirmed reservation. Frontier is a very good airline, but they have a limited service area.

3. the ones you mention, in my opinion, are average. The ones I feel have given very good service, and I have flown them are Cathay Pacific and Midwest.
